M.A.N.U.P. Rehabilitation Services case management and counseling is designed to support and assist participants to successfully reintegrate into the community as they move through the process from crisis to hope, into self-sufficiency, independence, and stability. It is also designed to connect them with networks of support both in transitional housing and upon exiting the program. The purpose of case management is to assist participants in achieving stability and prepare them for independent community living. Participants and their Case Manager collaboratively develop individual service plans, which specify participant needs, service goals, and the services and resources desired. The case manager provides information, referrals, support, and advocacy in the areas of housing, employment, medical care, finance, and other areas as needed. This is accomplished from a three-tier approach:

• Participant centered – participants are collaboratively involved in the case management process and take the lead in developing their service plan.
• Strengths-based – case management focuses on and builds upon participants strengths, skills, assets, and resources to overcome their barriers in preparation for fully living independently.
• Meeting appropriate needs – Case Manager works with participants to build life skills, reduce recidivism, problematic thinking and behavior patterns, obtain and maintain living wage employment, and positive connections with family, peers, and the community.
